$23.16 Job Description The Behavioral Health Recovery Services
Community Case Manager (RSCCM) serves as a member of a
multi-disciplinary patient care team for NATIVE HEALTH’S Recovery
Services/Behavioral Health Division. The RSCCM provides case
management services that support the treatment goals of the
patients' care plan. The Recovery Services Community Case Manager
represents the company professionally and positively to enhance and
promote the core values and mission of NATIVE HEALTH, always
exercising utmost discretion, diplomacy and tact in customer
transactions. KEY ACCOUNTABILITIES: Provides case management to
persons who are experiencing homelessness, domestic violence/sexual
assault, human trafficking and may be in the following settings:
Encampments, on the street, survivors of the fraudulent sober
living homes or at risk of homelessness. Supports the Recovery
Support Team and the Domestic Violence and Prevention Case Manager.
Participate in quality improvement and quality assurance activities
for the department. Supports the organization as needed. KNOWLEDGE,
SKILLS AND ABILITIES: Knowledge of case management practices.
Foster a person-centered approach and maximize patient
self-determination. Demonstrates respect for the patient’s
preferences, interests, needs, culture, language, and belief
system. The ability to work independently and successfully balance
time in the community with clients as well as with community
partners. Positive attitude and the ability to advocate on behalf
of the patient’s needs. Knowledge of existing social and behavioral
health resources for patient care. Excellent written and verbal
communication skills and ability to establish rapport. Good
documentation skills, including detailed and accurate
documentation. Mental health/substance abuse terminology knowledge
and understanding. Knowledge of mental health issues of the
populations served. Ability to work independently with a strong
sense of focus, task-oriented, non- judgmental, and clear sense of
boundaries. A strong sense of and respect for confidentiality.
Courteous, honest and professional always. Customer service
oriented. Ability to plan work and carry out assignments
effectively Excellent computer skills with Microsoft Office and
electronic health record. Remain current and complete assigned
Relias Trainings by deadline. Qualifications Bachelor’s degree in
behavioral health or social services related fields strongly
preferred (or experience equivalent). Master’s degree is preferred.
Two years of related case management experience is required.
Experience working with underserved and diverse populations is
preferred. Experience in NextGen EHR preferred. Standard Employment
Requirements: Must possess a valid and unrestricted Arizona
driver’s license (within 30 days of hire if applicant is from
out-of-state) with no DUI/DWI or reckless driving convictions in
the last five years. No more than two at-fault accidents in the
last three years. Must maintain a valid Arizona driver’s license
during employment. Must be insurable under NATIVE HEALTH’s
liability auto policy. Must pass a criminal background check and
obtain a Class I Fingerprint Clearance Card within the initial
ninety (90) calendar days of employment and retain a valid card
while employed with NATIVE HEALTH. Must pass substance abuse
testing upon employment and submit to random testing during
employment. Must provide an updated immunization (IZ) record at the
time of employment. Must have a current (within 12 months)
Tuberculosis (TB) skin report upon employment and provide an annual
update. Must obtain Basic Life Support (BLS) certification within
(90) calendar days of employment and maintain certification during
employment. Additional Information Native Health complies with all
state and federal requirements, including EEOC requirements.
