Physician provides medical services within the scope of their
licensing, training, and clinical privileges, and supervises and
teaches residents and/or fellows as indicated. Education and

Successful completion of a residency program. AZ MD License Board
qualifed or certified Credentialed SJHMC Membership in appropriate
